The Music Room cafe in Sun Street was given the score after assessment on April 5, the Food Standards Agency's website shows.
Businesses are rated from 5 to 0 with a 5 score being the highest.
A three rating means the business was found to be generally satisfactory.
Of Lancaster's 213 restaurants, cafes and canteens with ratings, 131 (62%) have ratings of five and none have zero ratings.
